In the event you experience an injury not necessitating immediate medical attention at work, please notify your supervisor and the HR office as soon as possible. We will guide you through the process and make sure you are provided the care you need. Below are some documents and resources to use in case of injury at work. 

If you need to be seen by a doctor for this work-related injury, you will need to go to a Concentra location, not your usual doctor. Here is a list of San Diego Concentra locations. You will need to bring the completed Treatment Authorization Form with you and do not need to make an appointment.
